ADHD Isn’t New. We’re Just Finally Paying Attention.

One of the biggest misconceptions about ADHD is that it’s some sort of modern phenomenon. It’s a product of smartphones, social media, or children eating too many blue sweets and watching too much television.

It isn’t.

ADHD has existed for as long as humans have been trying to understand the human mind. What has changed is our language, our research, and thankfully, our willingness to be curious rather than simply judgmental.

And honestly, if you look back at the history of ADHD, it’s a fascinating lesson in how society has often mistaken neurological differences for moral failings.

For a long time, people weren’t asking, “How does this brain work?”

They were asking, “Why won’t this person just behave?”

Those are two very different questions.

One of the earliest descriptions that resembles what we now recognise as ADHD came from the Scottish physician Sir Alexander Crichton in the late 1700s. He described an “incapacity of attending with a necessary degree of constancy to any one object.” In other words, he observed people who struggled to sustain attention, not because they lacked intelligence, but because their attention seemed difficult to regulate.

That word, regulation, is important.

Because, despite the name, ADHD is not simply an attention deficit. If anything, many people with ADHD would tell you they have plenty of attention. The challenge is deciding where it goes and whether they have any control over it. Sometimes attention disappears completely. Sometimes it becomes hyper-focused on reorganising the spice rack instead of answering emails.

The brain has its own agenda.

By the early 1900s, British paediatrician Sir George Frederic Still described children who struggled with impulse control and behaviour despite appearing intellectually capable. He referred to this as a defect of “moral control,” a phrase that sounds incredibly uncomfortable through today’s lens.

Thankfully, we’ve moved away from the idea that neurological differences reflect someone’s character.

Because being impulsive is not the same as being immoral.

Being distracted is not the same as being lazy. And struggling to regulate attention does not mean someone lacks intelligence or motivation.

In the 1930s, an American psychiatrist named Charles Bradley made an unexpected discovery when children who were given stimulant medication actually improved in their ability to learn and function at school. It was a surprising finding because stimulants might seem like the last thing someone who appears “hyperactive” would need.

Unfortunately, like many important discoveries throughout history, this observation wasn’t fully appreciated at the time, and much of the research faded into the background for years.

Classic humanity, really.

It wasn’t until 1952 that conditions resembling what we now know as ADHD began appearing in the Diagnostic and Statistical Manual of Mental Disorders, commonly known as the DSM. Even then, understanding was limited, and the focus remained almost entirely on children.

For decades, ADHD was largely viewed as something boys had. Not girls. Not adults. Just boys.

Certainly not the woman quietly sitting in the corner who spent the entire meeting thinking about seventeen unrelated topics while smiling and nodding.

As research developed, so did the terminology. The condition was called Hyperkinetic Reaction of Childhood before becoming Attention Deficit Disorder, or ADD. Researchers believed hyperactivity wasn’t present in everyone, so they recognised presentations with and without hyperactivity.

Then, in 1987, the name changed again to Attention Deficit Hyperactivity Disorder, or ADHD, bringing together symptoms of inattention, impulsivity, and hyperactivity.

By 2000, the DSM recognised three presentations that many people will still be familiar with today: predominantly inattentive, predominantly hyperactive impulsive, and combined presentation.

Personally, and this is simply my opinion rather than a clinical fact, I think ADHD is a rather unfortunate name.

Because it suggests people have a shortage of attention when many of us feel like we have an abundance of it. The difficulty is regulating where that attention lands and how long it stays there.

It’s less about having no attention and more about having a very enthusiastic Labrador puppy deciding where everyone’s going today.

Research also continues to show that ADHD involves much more than concentration. Executive functioning, emotional regulation, sleep, motivation, working memory, and sensory processing can all be affected.

Many people experience difficulties with sleep because the release of melatonin, the hormone that signals it’s time to sleep, can occur later than in people without ADHD. Others struggle with regulating temperature, managing emotions, organising tasks, or switching attention between activities.

The common thread isn’t laziness. It’s regulation.

Current evidence suggests that ADHD involves differences in the brain’s dopamine pathways and reward systems. Dopamine plays a significant role in motivation, learning, attention, and pleasure. When those systems function differently, the brain naturally seeks stimulation to help maintain engagement and regulation.

Suddenly, the endless search for novelty, excitement, urgency, or even scrolling for “just five minutes” starts to make a little more sense.

For a long time, professionals believed ADHD was something children simply grew out of.

Now we know that’s largely untrue.

Children grow into adults.

Their brains don’t magically receive a software update on their eighteenth birthday.

What often happens instead is adaptation.

Many adults become experts at masking. They develop elaborate coping strategies, people pleasing behaviours, perfectionism, anxiety-driven productivity, or sometimes less helpful ways of self-regulating through substances or other dopamine-seeking behaviours.

From the outside, it can look like they’re functioning perfectly. Inside, they’re exhausted.

This is one reason why so many adults receive diagnoses later in life and experience enormous relief alongside grief. Relief because things finally make sense. Grief because they’ve spent years believing they were simply failing at being a person.

Research has also shown that ADHD is highly heritable, with genetics playing a significant role. If one family member has ADHD, there is a much greater likelihood that other relatives may also have it.

Which creates an interesting situation.

If everyone in your family loses their keys, interrupts each other mid-sentence, starts seventeen DIY projects simultaneously, and somehow considers cereal an acceptable dinner three nights a week, you might assume that’s just what being human looks like.

It’s only when you step outside your family system that you realise not everyone lives quite like this.

We’re remarkably adaptable creatures. We normalise our experiences because they’re all we’ve ever known.

More recently, researchers have also been exploring how hormonal changes can influence ADHD symptoms. Puberty, pregnancy, postpartum, perimenopause, menopause, and menstrual cycle fluctuations can all affect executive functioning, memory, emotional regulation, and attention.

Many women discover or seek assessment for ADHD during these life stages because the strategies they’ve relied upon suddenly stop working.

Again, the condition isn’t new. Our understanding is.

Sometimes people ask whether ADHD existed throughout human history. There are interesting theories suggesting that traits associated with ADHD may have been advantageous in hunter-gatherer societies. Curiosity, rapid scanning of the environment, creativity, novelty seeking, and adaptability could all have contributed to survival.

Of course, we can’t travel back in time to confirm this.

But it’s an intriguing way of thinking about neurodiversity. Traits that may feel challenging in one environment can become strengths in another.I don’t personally think ADHD is a superpower. Because living with ADHD in a world largely designed for neurotypical brains often feels frustrating, exhausting, and confusing.

It can feel like everyone else attended an orientation session called “How to Be a Functional Adult,” and somehow you missed it because you got distracted by a squirrel on the way in.

But equally, I also don’t believe ADHD is a flaw.

I’ve seen extraordinary creativity, empathy, humour, resilience, intuition, innovation, and passion within the ADHD community. Those qualities deserve recognition too.

ADHD does not mean you’re careless.

It does not mean you’re stupid.

It does not mean you’re lazy, selfish, irresponsible, or incapable.

It means your brain processes information differently.The more we understand that difference through research, compassion, and genuine curiosity, the less time we spend blaming ourselves for struggling in systems that were never designed with our brains in mind.

ADHD has always been here. Thankfully, so has our capacity to learn.

We’re just finally starting to ask better questions.
